Breaking Down the Basics: What are Biosimilars and How Do They Differ from Generics?

Biosimilars and generics both play an important role in expanding access to affordable medicines, but they differ significantly in structure, manufacturing, and regulatory approval. Understanding these distinctions is essential for healthcare professionals, policymakers, and patients navigating treatment choices.

Generic medicines are chemically synthesised compounds that are identical in composition to their originator drugs. They contain the same active ingredients, are delivered in the same dosage and form, and must demonstrate bioequivalence—meaning they perform in the same way in the body as the original product [1][2]. Their approval is based primarily on pharmacokinetic studies rather than new clinical trials.

In contrast, biosimilars are complex biologic products developed to be highly similar, but not identical, to a reference biologic that has already been approved. Because biologics are large protein-based molecules produced in living cells, minor variability is expected, though it must not affect safety or efficacy [3][4]. Biosimilars undergo extensive analytical, non-clinical, and clinical studies to demonstrate that any differences do not result in clinically meaningful outcomes [5].

The regulatory pathways reflect this complexity. Generic medicines follow an abbreviated registration process focused on demonstrating equivalence, whereas biosimilars are subject to more rigorous assessments that include comparability studies and immunogenicity evaluations [6]. This distinction influences the time, cost, and evidence required for market approval.

In South Africa, the South African Health Products Regulatory Authority (SAHPRA) developed specific guidance for biosimilars in line with international standards. The SAHPRA framework, first released in 2014 and updated in 2025, outlines quality, safety, efficacy, and risk management requirements for biosimilars [7]. In a more recent development, SAHPRA has begun recognising biosimilars as interchangeable with their reference products under certain conditions—supporting broader adoption and market confidence [8].

The logistics of handling these products also differ. Generics are generally more stable and do not require cold-chain storage. Biosimilars, on the other hand, often require refrigeration and sensitive handling due to their biologic composition [9].

Cost is another important factor. Generic drugs typically offer price reductions of 80–90% compared to brand-name equivalents due to lower development and manufacturing costs [10]. While biosimilars usually achieve more modest cost reduction, often in the range of 15–30%, they still provide meaningful savings for high-cost biologic therapies used in areas like oncology, rheumatology, and diabetes care [11][12].

By broadening access and encouraging competition, both biosimilars and generics have become integral components of sustainable healthcare systems. For countries like South Africa, where medicine affordability and system equity remain national priorities, understanding and leveraging these alternatives is a vital part of long-term health planning.
